Employee performance review forms

Hi everyone...totally new to flow and forms...anyone ever tried to make a form for employee performance reviews?

    hey @CollabTechie, Its basically to replace the old-school and printer heavy activity of printing A LOT of papers. Every end of the year, all (300+) employees get their performance evaluated by their managers - all that used to go on a paper which then got manually approved (1st level) by the head of their department  - then, it goes to me (2nd level approval). After this it goes to the 3rd level of approval - which is also allowed to modify the final score. 

     

    The end goal is to smoothly shift this old practice to the digital world. 

     

    So far...I've managed to tinker around to make a pilot form on my own... and I'm in the process of learning how to set up a proper sharepoint list for the responses. It's a bit tricky as not all employees have 365 profiles ( but all the department heads do! and most of the managers as well....

    For MS forms, is there an easy way for employees just to fill in their "ID number"...and once I submitted, it automatically looks up something like a list or excel sheet to pull up details linked to that ID number..like name...department...level...and more importantly...the head of their department ( the 1st level approval )?

    Hi, I´m basically facing the excat same challenge, so I´m curios if you actually ended up using a flow and how it´s working out? My additional question is: by legal obligation, the employee and manager should receive a copy of their answers. Any chance to automate that as well? Iwould be thankful for any hints... Thanks in advance , Annika
