Issue - Flow creating two Line Items in SharePoint List

Has anything changed in the past day or two? We have had a flow running without issues for months. This morning, it started creating duplicate line items in SharePoint. The last modified date was 3 months ago. What could cause this to happen?

    Today, 7/30/2024, Microsoft has been experiencing some network infrastructure issues, which have also impacted the Power Platform service. This might be related to the behavior you are experiencing, but it’s hard to tell.

    Microsoft service health status (cloud.microsoft)
    Azure status
