Form reponse to word doc and convert to pdf

Hi I have a form and I want the response to be filled up to the word doc which has plain text and input certain cell into the word doc. Then convert to pdf. How do I do so?

    Convert word to pdf is easy, just need to follow the steps detailed on this post:

    Not sure I understood your first requirement, though. Are you talking about a Microsoft Forms survey?

    On this second post, you will find official documentation describing how to build a Word template and then fill the template fields with selected dynamic values to generate a Word document, by means of Word Online connector .
