Flow working unless the form that starts the trigger is filled in too quickly, have I missed a step?

Hi

 

I am new to Power Automate so I apologise if this question seems obvious or it's been asked before but I have noticed an issue with the flow I have created.

 

My flow is triggered when a Microsoft form is filled in and submitted, it starts by adding a line to an excel sheet and then sends approval for the information that has been added. This all works perfectly well but I noticed when testing it that if I filled in the form and submitted it then quickly submitted it again it sometimes seems to be jumping the step of adding a line to the excel spreadsheet, it does follow the rest of the process and submits the approval though and doesn't show as a fail. Is there something I can add to stop this from happening?
