Hello! I'm one of the developers of flows in our company and we started having this problem about 2 weeks ago.
We developed an approval system for price set-up requests last year in which a certain department submits their requests via SharePoint list. An approval flow will be sent to the corresponding approvers throughout the transaction.
The whole system works fine until 2 weeks ago, some approvers were complaining that some approval flows were not being sent to their Outlook and to their Teams.
- No changes were made to their Outlook and Teams settings recently (we already did the whitelisting(?) of Microsoft Power Automate as sender).
- They've checked their junk and spam
- Some days have already passed but the approval flows were not yet received in both Outlook and Teams
- In the flow, we set up an action that will send an email notification BEFORE the approval action. They receive the email notification only.
- When we check the history run of the flow, it only shows as this (see 'Apply to each 19)':
- We tried cancelling the flow and resubmitting it and surprisingly, the approvers would then receive the approval flow in both Outlook and Teams for that specific flow only that was resubmitted.
- The unusual thing is that this they said this started happening 2 weeks ago and happens only to some requests and only to some approval actions (ex. approval action in the first part of the flow or approval action in middle/last part). I can't pinpoint it to only one approval action.
Anyway, what happens now is that if there's a request that that they need and they noticed that they haven't received the approval flow for some time, they'd always have to ask somebody to resubmit the flow.

Maybe check your Connections page in Power Automate to see if any connections need to be fixed (Microsoft Dataverse,etc.).
