I set up the flow as below to Send Approval for Microsoft Forms and map to Microsoft List for the approved responses with the file attachment.

However, in order to upload file attachment into List item, I have to create item first, regardless approved or rejected. I tried to create another item after approval into a second List, but the approved response does not go in and the first list shows both approved and rejected & the approval status does not update into the first list after approval.

Please advise how i can update the flow to achieve this.
