Flow to populate data from Form to Sharepoint List

I am very new to this - this is only my second flow and I am sure there is a very easy answer.  It is a very simple flow with a very simple form / list.  I have looked up a lot of the forums looking for a solution - however most of the responses are above me.

 

I am trying to populate a Sharepoint list from data within a Form.  The flow works if all fields are completed but if either of these 2 fields are left blank, it fails.   I expect the majority of responses will leave these 2 fields blank which is fine. I get this error message which I believe relates to the empty fields.

    Hi Marco

     

    Thanks for your response.  However I still don't seem to be able to get it to work.  This is my actual flow

    When I try and add the Initialize Variable before Create Item,  I get an error saying it can only be added at the top level.

    What am I missing?

    Hello @MerlinT ,

    please initialize the Variable at the beginning of your flow.

     

    Then, in the below steps you can set it based on the Condition.

    Hi Marco

     

    Thank you so much for your time.  After spending way too many hours on this, I was able to fix it by simply changing the column format in my sharepoint list.
