Adjusting the approval request for a Forms form

Hello everyone,
I have the following problem:

Employees can fill out a Forms form that I have created. The completed form is to be sent to a manager for approval. The automatically generated mail that is sent to the manager looks like this:

There is one line "created by" which shows my email adress because i created the forms. The line "requested for" shows the Mail adress of the person who submitted the form. In my opinion, this is kind of confusing for the manager. Is it possible to delete the line "created by"? Thus the Mail is auto generated i couldn't finde a way to make it. Maybe somebody knows.
