How to collect data from custom actionable mesages?

I built a custom actionable messages where it is suppose to collect data based on the user selections (Yes or No) and reasons.

This is my Flow template, how do i get the data of users selecting Yes OR No and the reasons?

    You need to create another flow and use When an HTTP request is received as the trigger to receive a response.
