I want as situation whenever i click on the + icon it will show another text box and drop down.

I think you will need to leverage on a Gallery, each time you clik on + icon you add an item in a collection that is bound to a gallery and the gallery displays textboxes.

Yes sure.
So this is the + Icon onselect event
The gallery items are bound to that _ collection
and just for debugging I added a label that is binded to the item id
i have been able to add whatever i capture into my gallary form but i am unable to update my SharePoint using the OnSuccess.
