After shelving some PowerApp work for a few months, I am revisiting and still trying to figure out how to make this work... How to submit the contents of a PowerApp form via the SubmitForm function using the "Concatenate" command... Attached are screen shots of the PowerApp and its four "screens" for reference... below is the fx command contents triggered by pressing the Submit button on the form, for review...

PowerApps gives error messages but does not seem to provide any further information as to the specifics errors found and makes it really hard to figure out solutions...
So any help from the community would be GREATLY appreciated!!
