Help with Error on Apply To Each - An action failed. No dependent actions succeeded

Hi all!

Stupid new at building flows with essentially no programming background. I'm trying to take form responses from an internal form (project authorization) and output on a Word Document for internal signatures. Ideally this would be on our letterhead template with signature lines already in it but I can surive withou that :). This file is then saved to our Dropbox and a copy emailed to the Project Controller.

My current workflow and error messages are below.

Hi @mhills91,

The "get response details" action run failed in your screenshot.

Have use choose the same form in the trigger and the "get response details" action?

Please make sure you have choosed the same form in the trigger and the "get response details" action.

Also please take a try to create a new flow or create a new connection to "get response details" action to see if the issue still exists.
