Unable to create a readable word Document from a Form

Hi everyone,

 

I'm trying to create a word document from a form, but I can only create unreadeable ones. 

 

Here is the flow:

And here is the 'completed' word doc:

Do you have any suggestions on how can I do a normal word document with all the datas in it?
