Microsoft Forms creating multiple response ID's for the same form submission

I'm not sure if this ties to my Power Automate flows but Forms is randomly creating multiple response ID's for the same form submission. I have a flow built to create a list item based on the Form response ID so multiple list items are being created for the same submission. 
 
for example, the submitter created this form submission just once, she only clicked submit once. Two response ID's were created at the exact same start time. 
 
Any thoughts on why this is happening? I didn't think at this point it's even connecting to my flow since the flow needs the response ID to trigger the flow. Is this a Forms problem?

    @Pstork1 Thank you, but I'm confused because I don't even think it's my flow that's causing this problem. The flow picks up the response ID from the form. The form is creating multiple response ID's for the same submission. The flow recognizes each of these response ID's and creates the list item for each which is expected functionality. I didn't think my flow is even an issue here because the Form is what is causing the issue of multiple response ID's. Do I not understand the process?

    @KH-21042011-0 Is the Screenshot you posted in the question a picture from the list? or from somewhere else?  I assumed it was from the list. If that is the case then something in your flow is creating two items for each form submission. That's why I asked to see the flow.  If the screenshot is from somewhere else, then where is it coming from?

    @Pstork1 Thank you, you had asked for screenshots of my flow so I added them. I still don't understand how the flow is triggered yet though if the triggers is tied to the response ID from the form and the form is creating multiple response ID's for the same submission. This is my question. I apologize if I'm not making sense.
     
    This is the detailed example:
     
    • Jane Doe creates a form submission, clicks Submit once. 
    • The synced Excel spreadsheet listing all the responses shows two identical submissions with almost exact same timestamps with individual response ID's, as in screenshot in original post.
    • My flow picks up the two response ID's and creates two items in my Sharepoint list, as is expected functionality.
    I don't understand why the form is creating two response ID's for the same submission.

    So the original screenshot is from the Excel tracking sheet for the form and not the list created by the flow. That is what my question was for.  The only answer I can come up with is that two people tried to submit the same form within a minute of each other.  So the Form generated the same responseID. That shouldn't happen, but it is possible.  Other than that I can't explain it.  The only thing to note is that although they have the same start time they have different completion times. But I don't have a better explanation than that.

    @Pstork1 Thank you for taking the time to respond to my post, I want to think it was just a random thing on a Monday, it's not happening today with my recent submissions. You confirmed that it isn't anything that has to do with my flow, which was my main concern. Thanks again
