I have multiple flows with Cognito forms which all suddenly started to fail.
This is the error I am receiving:
"Could not retrieve values. The dynamic invocation request failed with error: { "message": "Unable to match incoming request to an operation." }."
I had the same issue but, after refreshing the connector there were new settings at the bottom:
After setting those as shown, my flow works again.

I am in the same position with 4 flows. The error occurs at the Get Document stage. The document ID is returned correctly in the error message so some semblance of connection must be happening between PA and Cognito Forms.
The Cognito connection was flagged with an error. I reconnected but the flows still generate an error when I resubmit.
Same issue here in a Managed solution. I was able to get the flows running again by removing and re-adding the connector, but this is not a great solution as it deletes all the downstream usage of the variables. Hoping for a fix. I have submitted a ticket to Cognito in case they updated their connector.
