Is it possible to prefill checkboxes by using the LookUp function to a SharePoint List item?

Good day,

 

I'm curious to know if this is possible and how I would go about it?

I'm trying to mimic this particular aesthetic within a paper-form:

The idea is for a Microsoft Forms to act as a Front-end to register the choice within a Choice column in the SharePoint List with the column title 'Product Type'.

 

I've successfully created a text input whereby when a UniqueID is typed in and selected, the variable 'varUniqueID' is set to said UniqueID and the SharePoint list item is selected and printed into the text labels with the LookUp function. However, I can't seem to figure out how to go about this. Any ideas? That would be much appreciated. Thank you all in advance.
