I have the Dynamics 365 extension installed:
But when I try and connect PVA to Omnichannel, I constantly get this message:
Does anyone have any ideas on why this happens ?

Hi @Monkeyman,
Just to double check. Have you registered an application in Azure AD to which the omnichannel can be connected to? It is one of the prerequisites for hand-off configuration:
If you have that application id you should be able to connect via the steps below:
