Error: Resource Not Found- using ArcGIS/Survey123 Submitted Survey Trigger

Not sure why my trigger is not working. When I test it, it is just loading does not work at all. My code when editing states True, but my output when I try to start the flow brings up False.

    Not sure around the trigger- but looking at error message - i think form submitted details are deleted 
     
    Are you testing the trigger using previous trigger results ?
