Flow Sent Approval Email is sending twice.

Dear All,

 

I creating some flow for approval process by sending email, with 3 times approval procces. My flow is connected to PowerApps formulir and SharePoint database. When user is submit the form, the approval email was automatically sent to the 1st approver twice. Idk why. Here is my flow picture below. Please help me to solve it, thank you.
