"When an item is created or modified" from SharePoint not triggered for each modification version

I'm working on a complex Power Automate solution that sends approvals to different people in parallel and updates a SharePoint list item with each response.


I've found the "When an item is created or modified" from SharePoint connector is not triggered for each modification version when multiple modifications occur within a short period.

Versions 117, 120, 122, 123, 125, 127 and 128 never triggered the flow. This is the only flow configured to trigger on modifications of an item in this SharePoint list.

 

This is the recent version history for the list item:

The trigger doesn't have any conditions:

I don't mind if the the flow doesn't trigger when the only modification is the approval status changing to RequesterPending because I've added actions to compare the item with the previous version and cancel the flow early on if that's the only change.

 

When I look at "Failed checks" for the flow, there is nothing listed.

 

The solution isn't in the default environment.

 

Is there a limitation that explains why this flow doesn't trigger on every modification?
