This seems to have been raised before, but hasn't been solved.
After an approval workflow is run, approver receives two same approval emails within a few minutes.
After the first email is approved, the document status in SP is set to approved and in Power Automate the flow shows as successful.
The the second email is received. Now it doesn't matter if one rejects or approves it, nothing changes anywhere.
I have now tried to run a flow with a delay command (thinking, that maybe autosave triggers it), but same happens, two email are received.
Seems like a bug within flows that needs to be fixed.

Hello,
I have the same problem, and I don't have any other flows starting with the same trigger. My flow starts when an item is created in a Sharepoint list, then it sned an approval request. But, although I create only one element in the sharepoit list and the flow only has one execution underway, two approval processes are sent... Can you help me please?

I think there are two seperate flows that are running with the same trigger (When an email is received), can you check in your Flow environment if there is any other flow with the same trigger for the exact same mailbox.
This doesnt look like a bug instead multiple flows running on trigger for a same mail box.
