"Start and Wait for Approval Flow" not working

The last few months and weeks the Holiday Request Form works until recently. The approving managers are not receiving the requests although employees received confirmation notification that the requests have been submitted. I have checked my flow and found that the Get manager (V2) flow has failed. After a few days spent on trying to fix the Get Manager (V2) to work by replacing my "Responder's email address" (that is pulled through using dynamic content as part of the form questions) with "Mail", this fixed the error but after hitting the flow checker and running tests, the next flow: "Start and wait for an approval" is now showing an error. 
 
 
 
I am struggling to resolve this for I keep getting the below.
 
And when I tried following the link it always give me an error 404. I have been on the community site to check if there is any resolution to this but nothing came! Can anyone please help? This is a simple holiday request form with a two-level approval. Whether I input the 1.) Responder's email address (email address of the responder who submitted the form) and 2. Mail (under the Get manager profile (V2) of the dynamic content, on the Assigned to field, nothing works. 
 
The first one worked in the last weeks and even if I change it with latter, the result of the flow test for the start and wait for approval is still a fail (!)

    This error can occur if the user account in the "Assign to" has been disabled.
     
    Please check that the account has not been disabled in MS365/Entra ID.

    Thanks for the response Ellis Karim. All approvers have no blocked signs ins. I am working closely with our IT on this project. It has taken much of their time as mine figuring out how to resolve this process which is meant to be simple.
     
    When I hit Save > Flow Checker all seems okay. But when I hit Test, I can only do it manually for the automatic test says there are currently no runs for this flow. Although I followed the manual testing to get a response, (with 5 submitted responses), none of the responses even registered as a run. 
     
    I have asked IT to "Restore any deleted flows". Apparently, you can restore any flows within the last 21 days. I hope they can just restore the original one as it's only been two weeks. I have employees on my back asking when they can submit a request for holiday, thus the urgency of getting this sorted.
