multiselect check boxes in custom SharePoint form not retaining check box values after save

Created a form using checkboxes using a gallery and checkbox under a datacard
 
The form saves but when opening in the future I do not see any of my checkbox values checked.
 
Everything is cleared.
 
I even created a lookup to ID on FormScreen1, onvisible but that is not working either.

    Hi olsen9,
     
    Did you pass anything in the Item property of the Form? Make sure the Checkbox default items property is also set to the selected item's column.
