HI Team, 

Ive built this simple app. My Form pulls from Excel online.

When i try and open the APP, the below error messages are shown?

Any support please is always appreciated

    You'll have to share a bunch more info on your Excel sheet and the conditions using its data to nail this down for a solution. However, the errors indicate it is not reading what you expected from the Excel sheet and then your app logic fails as a result.
