Adding attachments from MS form with multiple attachment questions

Hi,

I am trying to work out how to add attachments from a form that has multiple questions that allow you to attach a doc, I understand how to do this with one question, but I am struggling with multiple:

I have built a test form to show you the format incase my description of the issue is not clear:

Any point in the right direction would be most appreciated :).

Hi @Jon8930 ,

You need to add as many condition blocks as questions in the survey if the answers are not mandatory:

You can simply copy the first condition control and paste in new steps, modify the question name in Condition:
