Power Automate MS Form Vendor creation in D365 F&O

I'm trying to create a Flow from an MS form for a vendor to be created in D365 F&O, my flow works as far as the condition, then it Skipped the branching conditions.

ActionBranchingConditionNotSatisfied

The execution of template action 'Create_record' skipped: the branching condition for this action is not satisfied.

Can any assist me, just keep going around in circles now, and not sure what I need to fix, any help would be appreciated.

@DonnaWarner21 it is likely that there is an error in your condition. Could you post a screenshot of that?

@DonnaWarner21 it should be Outcome is equal to Approve, not Approved.

This seems to have done the trick on the condition, but the Create record in D365, is now throwing me this, as a Bad Request.

Action 'Create_record' failed: { "@odata.context": "https://lrg-uat.sandbox.operations.dynamics.com/data/$metadata#Edm.String", "value": "Write returned RecId 0 for table row of type 'RetailVendVendorV3Entity'. Infolog: Error: Mandatory field 'Organisation name' not set.\r\n" }
