Employee performance review forms flow

Does anyone have a flow template for employee reviews? i need help in building one. 

The process:

A)

  • there would be a list of employees with the required date of reviews
  • submit the form to employees for fill up
  • submit the employee's response to the manager for review 
  • manager will fill up another form for comments and recommendations for a rate increase or whatsoever
  • submit both forms back to HR
  • the form will be saved to a folder

B) 

  • Send a word form to the employee
  • the employee will submit the form
  • the form will be forwarded to the manager for input (on the same form)
  • manager will submit the form back to HR
  • the form will be saved to a folder

and if there's a way to have the rating computed at the end. If this is not possible can anyone help me with a better idea? thanks
