Error while submitting forms

Hii ,
 
while submitting forms in PowerApps I am getting below errors. I have my database in Dataverse.
 
 
Kindly provide its reason and some solutions.

    It says that a value for the fiel 'de_name' is required, so you should try to add a field for name to your form. Or if the name field should't be used, then you should change the type of the name column (the primary column of the table) to autoincement. After it's changed to autoincrement the message should be gone.
