when using a form as a trigger, how to get a document uploaded to that form to be emailed automatically

Please help! 

 

I am really struggling. I have reviewed numerous blog posts, resources, and you tube videos for this and cannot figure it out.  I have a form as a trigger. That form has the option to upload a file. I need to have that file that is uploaded attached to an email later in the workflow. Below are pictures of the flow. What am I doing wrong?

    Please see the below screenshot. I keep getting a file not found error. I used a you tube video to come up with the file path. I have been struggling to figure out how to use the file path since I am trying to attach a document that was uploaded to a form that triggers my process. So I do not have the attachment today and am not sure how to point it to the correct file folder and response.

    Added one more screenshot to make sure I fixed my earlier issue with the append to array variable. Thank you by the way, this community is a life saver!

    Is this a compilation error or execution error?

    Also, I noticed that you are missing the Name property.
