Ideas for storing Quiz/Survey results in a manageable way

I have been researching for three days for a right solution for a person to view responses to a Form, that

  1. Doesn't see all the other responses.
  2. The reviewer can't edit the Form

Another requirement:

  1. Form questions and layout is changed often

So how to see the responses? Where I don't have to rebuild a dataverse entity or sharepoint list whenever there is a change.

 

Our set up is Dataverse and Powerapps. Teacher clicks button "Send test", trainee completes test. Teacher clicks the button "View test results". In their cell phones, ideally they could see the test results or get it sent to their email.

 

Saving the body of responses in a file is an option, but will not include the answers.

The people viewing it will not know what question is being answered. 

 

Has anyone found a way to have a way to more easily manage data? Maybe there is something better than Forms?
