Power Automate approvals blank, but Teams shows pending

I have an approval flow that still is running, though it is about a day away from timing out (past 30 days). The user cannot see any pending approvals in power automate, but in Teams the user can see there is a pending approval needed for this item. I am trying to understand how this could happen. Could it be due to the fact that this flow is very close to timing out?

Do you have multiple Environments? They might be looking at Approvals in their Personal Productivity environment but the Teams might be in a different Environment.

We do have more than one environment, but the users having this issue are in the correct one. This has happened several times now. A user will have 300-400 approval requests and they will get about a hundred completed and then the screen flashes and they all disappear. This happened with multiple users on different PCs. I can't understand what happened to cause them to disappear from power automate, but they do still exist in Teams.
