I create a separate list and make an app to Power Apps, I tried to run and test it but I get this error message when I submit a form in Power App. Although I dont have Title data in the Form

It looks like you have a SharePoint list. You have two options, Go back into SharePoint and change the Title field to Optional. Otherwise, in PowerApps you can select the field in the list of fields by checking it and then selecting the card containing the field removing the textbox control in the Update property of the card containing the Title field and replacing it with "Blah blah blah". (Keep the double quotes). Then hide the card by make its Visible property false.
@TebTim if your data source, for example a SharePoint list, has Title as a required column, you will need to have it on the form and enter a value. Either that or you could make the visible setting set for the title column in the form set to false and have a default text value. Either way if the column is required you need to save a value to the column.

I was wondering why I can't see the same form I get in Sharepoint list to the Power Apps. For example in Sharepoint I have Location, Business Phone data but in Power Apps I can't able to see it. Only few data are visible in Power Apps
@TebTim when you add a customized form from a SharePoint list, Power Apps doesn't always add all the columns. You just need to select the form then go to Edit fields and Add field to add any that you want which are missing.
