Very new to this, so bear with me! We receive an email response when we receive a newly completed patient details form.
This email provides patients name, email address and a yes/no response if they want to complete our feedback survey (we would normally email this survey link manually) hoping to be able to capture the data from the email received and automatically send them an email if they give a yes answer (with a delay).
The email received looks roughly like this:
You've received a new form for XXXXXX XXXXXXXX xxxxxxxxxx@live.com.au Consent given for feedback survey Yes |

You could create a Microsoft Form to structure the survey instead of Send the survey within an Email. Because the Send Email feature only supports gets a simple option response, but the patient's name, the email address couldn't do.
After you create it, you could send it by Email:
Then create a Flow as below:
