Unable to Connect MS Form in Team SharePoint for Power Automate Workflow

Hi All,

I'm facing an issue with connecting to a Microsoft Form that is saved in our team's SharePoint. I have owner access to the SharePoint site and am also added as a collaborator to the form.

This is for a workflow that creates a new line item in Microsoft Lists every time a new response is received from the MS form. The workflow works perfectly when the form is saved in my OneDrive, but I can't seem to connect to the form saved in the Team Group.

Any insights or solutions would be greatly appreciated!

Are you saying that the form name isn't populating in the trigger?

If so, click Enter Custom Value and paste the Form ID directly. You can find the Form ID in the URL bar.

For example, in the following sample form, the bolded text next to "id=" is the Form ID:
https://forms.office.com/Pages/ResponsePage.aspx?id=-Gvsjdhksjkjshdkjshdkjsdjsnnmdsmdbskbdkjsdskjdhskjdhkjsdhkjsdkjsdhskjhdkjs
