Status will not update from "Requested" to the outcome

Hi, I am having an issue where the status will not update from "requested" to "approved" or "rejected."  I am not sure what is going on, the flow seems to have just stopped updating the status.  It had been previously working as intended.  Is there something I can do to refresh?
