If one item was submitted, several item were created. How can I resolve it?

Hello All, 

 

Could you do me a favor? 

The flow works well, the issue is:

 

When one item was submitted, several items were created. Sometimes one, or 2 ~4 items were created. I don't know why. 

What I want is if one item is submitted, then one time is created. 

 

This is for coping one time and moving it to another folder within a list (Not library) 

This is my flow.

    Your "Get Items" must be bringing back more than one item, in that case. Can we see what filter you have on your Get Items?

    Thank you for your help!  The filter is 

    Title eq '@{triggerOutputs()?['body/Name/DisplayName']}'

    Do you have items in the list with duplicate DisplayNames?

    This is the list. The second one is the same. 

    Title can't be a people picker so I added 'Name' as a people picker.
