How we can extract text related to checkbox when a checkbox is checked from PDF

I am having PDF where multiple checkbox available. for each checkbox  there is text associated with it. So I need to extract text where checkbox is checked. At any point of time only one checkbox can be checked.

I have tried invoice processing and form processing from AI builder but always it extract the first checkbox text.

can someone please help me out ?

    Checkbox groups are not currently supported.

    The option here would be create one checkbox field per checkbox (IndividualCheckBox, CorporationCheckBox, PartnershipCheckBox, TrustCheckbox)

    If the text close to the checkbox is variable, then you should create 4 additional regular fields to extract the text.

    After the extraction, you could then reconstruct the checkboxes that were checked and the text associated.
