Hi,
Is there a way to add icons to form fields in a model driven app? My forms look a bit dull and very "texty". I would like to enhance the UX aspect without having to resort to a canvas app. Possible?

Do you want to add icon in a form in model driven app?
You just need to add Image web resources in your form.
I've made a similar test for your reference:
1)edit the form
2)switch to classic
3)insert->web resource
4)choose the image that you want in web resource
choose display label on the form
name for this web resource
You could set its size, layout, alignment...
5)after you add this successfully, save and publish this form
6)Then the icon will display like this in the form:
