I find a fix to my issue: the form being referenced was from a different connection and that person had changed their password recently.  This broke the authentication to the shared form.

I had to duplicate the form and re-link everything to the new form that was now associated with my account and everything worked.

I don't think this is a permanent solution, but works for now.

    Hi @Anonymous,

    What’s your current situation?

The error message says that the password has been changed. Please try to delete the connection then recreate with the updated authentication.

I assume it will work. Please take this doc for more details about Manage connections:

    https://docs.microsoft.com/en-us/flow/add-manage-connections

    Best regards,

    Mabel Mao

    Thank you!

Using the manage connection to reauthenticate the Microsoft Form connection solved my issue.
