Some controls have not yet migrated over to the Modern Form Designer yet. You will need to navigate to the classic interface to add a check box control. Note that a check box control can be configured with Yes/No choice columns only.
1. In the Form editor, click Switch to Classic.
2. Scroll down to the relevant tab, and then double click on the Yes/No choice field to launch the field properties window. Alternatively you can click on the field and select Change Properties from the Ribbon.
3. Select Formatting and select Two columns, then select Check box from the Control Formatting menu.
Click Save and Publish.
