When I use my submit form button, it replaces the recent list item in my sharepoint instead of creating a new one.
I have the default mode for my form as 'new' rather than 'edit'.

If you are opening a New form from a button, make sure NewForm(yourformname) is in the command sequence. Likewise, if you are opening the form from an item in a gallery, make sure EditForm(yourformname) is in the command sequence. Both these functions will override the default form mode.
If I refresh the app, it will add a new item to my sharepoint list. If I try to use my form twice without refreshing, it will just replace the item in sharepoint.
Maybe this is a better explanation of my problem.
SubmitForm(Change_Form) this is my 'OnSelect' for my submit button.
I have no idea what you mean by "refresh the app". The SubmitForm(Change_form), should not be activated unless you click it. Are you using a full app or just a SharePoint integration form?
