Embed Power Apps Screen link in Send an Email(V2) action

I am triggering my flow when a user changes the status of SharePoint list column to "Completed". I have create form and edit form in my Power Apps Canvas App. When the flow is triggered I would like to send the link of SharePoint list item to user. I dont want to use out of the box provided one "Link to item" with in the flow. I would like to send my Power Apps Screen link which is already working.

The link would be same for all the list items except the ID part.

Just that I dont how to embed this link in flow Send an Email function. Please help!
