Flow not working - Form to SharePoint list

My flow does not show any errors, however, when I submit a form, the data does not show up in my SharePoint list.

    Hi @kbeasley,

    Do you want to trigger a flow using the when a response is submitted and save data back to SP list?

    Could you please share the complete flow configuration?

     

    If the flow does not have any error, I would suggest you refresh the SP list and try to submit the form one more time.
