Retrieve Data From SQL and put it into form

I want to make a flow for a button in Power Apps to get the specific data from SQL and populate it into docx document. 

Is that any related article or tutorial related to this topic, I did some research but failed to find any similar scenario.

Is this flow is workable? Any help will be a great, thanks you.

    Is it the Populate Word Document and Create File is inside Apply Each? Because I seem cant get the result when I put outside of it.
