This app isn't opening correctly for a customized sharepoint form

Hi,

 

I have created a sharepoint form, customized it with powerapps and connected it to Microsoft flows. Once an item is created, the flow checks the email in "manager" field and grants access to this email ONLY to this item. 

 

When a user receives the link on his mail inbox and clicks the link, he gets an error "this app is not opening correctly and asks him to request access". 

 

What is wrong did I do there?

    For users to access any item on a Sharepoint list, the list must be shared with them with at least read-only level access.

    I am sharing the item already with them with edit permissions. it works fine if I am not connecting it to powerapps .. once I do, the item links keeps loading then show the error message.
