Unable to add a Power App to a Teams Channel

When trying to add a Power App to the 'General' channel of a Team, the loading spinner appears to be off center, then after a while I get this message.

This is a major issue for us as we have a Power App that uses Teams URL parameters. Anyone ever come across this? It only started happening last week.
