My flow is not triggering (sometimes)

Hello, I have created a flow on power automate that triggers when an item is created in a Microsoft lists site, I noted that sometimes and in a few cases, the workflow does not trigger, I checked the list and it contains information in the mandatory fields so I don't know if the missing trigger can be at a very specific time Power Automated is at max capacity or something like that, if needed I can post the complete workflow for suggestions that I didn't see when creating it as I'm not an expert on this, I just had an idea and I developed it with my basic computing skills.
 
Another question I have on mind is, is there a way to update the 28 day policy of the workflow to avoid the timeout? it's very rare that the process that this flow is intended to help take longer than 28 days but when it happens I have everyone asking why they did not receive their approvals notifications lol.

    One simple way to extend your approvals is to create a separate flow for each approval. This then allows each approval to take 28 days. It looks like you have two approvals, so this would effectively give you 56 days. I don't have time for a long explanation, so hopefully a brief explanation and a couple screen shots will help:
     
    To call one approval from another workflow, you use the When an HTTP request is received. You want to be sure to pass it the ID of the related SharePoint item so you can use a Get item action to get the data from the item:
     
    In my flows I configure reminders and handle the 28-day timeout by considering it a rejection for most solutions. Sometimes the process owner will want to do something else, but the majority are good with considering it a rejection:
     
    This is how the HTTP request at the bottom is configured to call the next approval:
     
    The example I used has five approvals, so if everyone takes 27 days to approve, the process could take 135 days. I didn't take a look at the link Anthony posted, but with this method if you include accounting for the time out, you could call the same workflow to start the approval over again and start another 28-day cycle. I have not done that for anything yet as most people agree that would end up causing confusion and nobody really wants to extend one approval beyond 28 days.

    Are you using trigger conditions in “When an item is created”? How many items are added per day? It might be a throttling problem.
