Creating Document Change Request Form

Dear All,
 
I am trying to create Document Change Request Approval process flow, steps would be as follows
 
1. User fills form in Forms
2. Request goes to Manager for approval
3. Upon Approval, received to particular email address to create the form
4. In case of Rejection, the Manager will write a comment and it goes back as email to user

    This really requires you to get yourself a consultant who knows how to build these. This is too much for us to build for you or to make decisions based on a business and process we don't understand.
     
    If you get stuck I'm happy to help, but I definitely don't feel comfortable myself to express the details in here. I do hope you understand.
