Flow triggering sporadically on new Form submission

I have 3 flows that have been running for months, they are triggered when a different Form is submitted. On auditing actual responses versus the number of times the flows are triggered, we are only seeing them triggered about 50-60% of the time. These forms can be run several hundred times a day. Any help would be appreciated as the data being created is heavily relied on & we have some pretty upset users now.

Maybe you could try to delete the connections and recreate them to see if that solves your problem.
