Flow Error: Skipped Tigger Event

I am running into a situation where the flow is erroring out, but not providing much information to trouble shoot. The error appears to be driven by the flow skipping over the initial trigger event (when SharePoint List Item Created or Modified) if I am understanding correctly. This flow does have some successful runs in between errors as well. And the successful runs align with the new items/changed item event I know of. These failed events are not coming from any trigger that I can pinpoint.

 

I am not sure where to begin on this, so appreciate any direction offered. Please let me know if I can provide any further information to troubleshoot.

Flow Design:

The flow is supposed to initiate whenever a list item is created or modified and simply copy a calculated column to the Title column and a Lookup column to another column.

    Hey @snwboarder159 Hope you are doing well.

     

    Are you running the Process Manually?

    Delete the Trigger and again use that trigger after that also if the problem not solved then Create new connection of the SharePoint and use that connection in your workflow.
